What does an elevated an DFI mean?
High sperm DNA fragmentation index (DFI) is associated with decreased fertility and poor outcomes in fertility treatments, particularly in insemination and IVF-treatments. It may also increase the risk of miscarriage. Spermatogenesis (maturing of sperm) takes some 3 months, so even with optimal lifestyle management, the improvement in DFI can only be apparent after about 4 months.
Lifestyle questionnaire and follow-up
The laboratory that performs the DNA fragmentation (DFI) analyses wishes that you would answer a questionnaire concerning your lifestyle. Stop smoking and the use of any nicotine products
Smoking as well as use of all other tobacco and nicotine products is highly detrimental for the sperm DFI. Smoking or use of these other products should be discontinued. Discontinuation is one of the most effective ways to improve the DFI value. Appropriate assistance and support should be available as required for the withdrawal.
Obesity and weight management
Obesity apparently effects testicular function and DFI through several mechanisms. The testicular temperature may be elevated causing sperm production to be compromised. Fat tissue slows the pituitary function and hormone secretion causing changes in testicular function. Intentional weight loss of 5-10% of is considered beneficial for improving DFI.
Diet
Appropriate diet typically requires attention so that obvious problems can be fixed.
- Eat plenty of vegetables.
- Reduce the consumption of red meats sugary foods and drinks.
- Favor vegetable fats fish and nuts.
- Minimize alcohol consumption.
Exercise
Sufficient physical exercise is considered important in efforts to improve the DFI, aim for 30 minutes daily. According to the latest studies it appears that high intensity training (HIT) may be more efficient than aerobic training. Exercise alleviates insulin resistance.
Antioxidant products
The benefits of antioxidant product remain unproven. When antioxidants and/or other vitamin or supplements are used it is important to stick to the recommended dose. Overdosing can be detrimental for DFI.
Medical evaluation
Your doctor can evaluate whether the abnormal DFI is due to varicocele, insulin resistance or other hormonal abnormalities. It is important to discuss the DFI finding with your fertility doctor when planning fertility treatments.
